Two former social workers who were assigned to the household have also been charged in the tot’s death and a secret investigative grand jury is being empaneled this month to look into what Brooklyn District Attorney Charles Hynes has called “systemic failures” at the city’s Administration for Children’s Services.

According to the arrest report, Camacho said the boy had been vomiting before he set him in the shower. He later found the boy collapsed in the tub.
Police later said Camacho had changed his story, saying that the boy had been injured by one of his brothers.
Dean and the others were part of a cabal led by Stephen Sims, 57, of San Leandro, Calif. Sims was the self-professed “den mother” who vetted people wanting to join the Internet group and had them send him images and videos of child pornography to prove they weren’t police officers. The group used social networking sites to exchange images and videos.
